Sunshine Children’s Home is a renowned 5-Star Pediatric Skilled Nursing Facility, 127-bed Sub-Acute/Pediatric Long-Term Care Facility located in Ossining, NY.
We specialize in providing care and treatment for medically complex children who require post-acute rehabilitative care, ensuring a nurturing environment where children can thrive.
*Position Summary*
Sunshine Children’s Home is seeking a compassionate and experienced Pediatric Dietitian with a strong comprehension of the principles of clinical practice, and knowledge of acute and chronic illnesses to provide nutritional assessment, monitoring, and support to children and young adults with complex medical and developmental needs in our pediatric post-acute care setting.
*Essential Responsibilities*
· Candidate must be able to understand and assess nutritional needs of medically complex population,using specialized knowledge and skills to optimize nutritional status.
· Conduct comprehensive nutritional assessments and develop individualized nutrition care plans.
· Monitor nutritional status, growth, weight, hydration, and laboratory values.
· Provide recommendations for oral, enteral, and specialized nutritional needs.
· Collaborate with physicians, nursing, speech/feeding therapy, and other members of the interdisciplinary team.
· Participate in care planning and communicate nutritional recommendations with families and caregivers.
· Provide nutrition education to staff, residents, and families as appropriate.
· Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements.
· Participate in quality improvement initiatives related to nutrition and resident outcomes.
*Qualifications*
· RD credential from the Academy of Nutrition and Dietetics or RD eligible. Licensed in New York State-Certified Dietitian-Nutritionist is preferred.
· Current registration with the Commission on Dietetic Registration (CDR).
· Pediatric nutrition experience preferred.
· Prior experience in post-acute care, long-term care, or complex pediatric care is a plus.
· Strong communication, organizational, and interdisciplinary collaboration skills.
· Commitment to providing family-centered, compassionate care.
